🌼 Band 7 RNLD Responsibilities:
As a Band 7 RNLD, you will play a pivotal role in the care and support of individuals with learning disabilities (LD) who are referred for NHS Continuing Healthcare (CHC) Funded Care & Nursing Care.
Leading and facilitating MDT assessments and care planning in accordance with the National Framework for NHS Continuing Health Care & Funded Nursing Care.
Conducting annual or as required reviews of LD CHC caseloads to ensure that current NHS CHC funded care meets the assessed primary health needs.
Case management and undertaking reviews of service users, ensuring quality, safety, and necessary updates to care plans and safeguarding processes.
Ensuring care is safe, least restrictive, and complies with the principles of the Mental Capacity Act (MCA) and relevant local processes.
Promoting communication and partnership working between statutory, independent, and voluntary sectors.
Providing relevant information and support on continuing healthcare to service users and carers.
Collaborating with CSU to commission and broker assessed primary health needs in accordance with national frameworks and local guidelines.
Promoting Personal Health Budgets and local funding pathways.
Attending meetings within the CLDT and/or CCG/CSU as required.
Liaising with other health professionals on clinical matters during the review process.
Maintaining professional confidentiality in all matters relating to patient care.
Demonstrating proficiency in IT skills, including Word, Excel, and RiO.
⭐️ What do you need to apply?
NMC Registration as a RN (Learning Disability Nurse) with recent experience in mental health, learning disabilities, and autism services.
Excellent knowledge of complex health needs associated with the LD client group.
Recent work experience in the implementation of the National Framework for NHS Continuing Health Care & Funded Nursing Care.
Ability to implement the National Framework for NHS Continuing Health Care & funded nursing care and support colleagues within the LD CHC Review Team.
Proficiency in coordinating and reviewing complex physical health care packages in accordance with national frameworks.
Ability to recognise and implement safeguarding processes and act within the principles of the MCA.
Capacity to manage caseloads independently and work effectively with families, clients, and other sectors.
Excellent IT skills and ability to communicate effectively with various MDT members within and external to the team.
